Choose all that apply. Your preparer may request additional records.

Gather only the records that apply to your situation. Checking an item here does not send it to STS.

GOOD TO KNOW

Before your appointment

Can I use your services virtually?

Yes. Start with the intake form and contact STS for appointment and secure document-transfer instructions.

How much will preparation cost?

Your fee depends on the services and complexity of your return. STS will explain the quote and payment terms before preparation begins.

Will these checkmarks update my client file?

No. They help you stay organized on this device. Your intake submission goes through Jotform, and the STS team confirms your official status separately.

Does intake mean my return has been filed?

No. Intake starts the process. Your return must be prepared, reviewed and authorized before transmission. Federal and state acceptance are confirmed separately.

What if a form does not open?

Contact STS by phone or email for help. Do not send sensitive tax information in your message.
